Co-pays are due at the time of service. We accept Cash, Checks, Money Orders, Visa, MasterCard, American Express and Discover for all payments. We will also take credit card payments over the phone.
Disability Forms and income insurance forms will require a $25 fee for completion by your provider. Please allow 5-7 business days for completion.
Records Requests We will send your records at your request to another physician's office as a courtesy. Individuals requesting records will be billed $25. Disability and other insurance requests will also be billed $25.
Prescription refills. Please contact your pharmacy and they will fax us a refill request. Our fax number is 360-433-6159. We do not refill prescriptions at nighttime or on the weekends. Please allow at least 24 hours turn around time.
Appointment cancellations. Please call as soon as possible if you need to cancel an appointment. No-show appointments could result in possible termination from our practice.
Late Patients After 15 min late your appointment may be rescheduled. New patients that do not have pre-registration information completed before scheduled appt time may also be rescheduled.
Cascadia Appointment Cancellations Due to the nature of our practice your appointment may be canceled or rescheduled due to emergency OB call and emergency surgeries and doctor scheduling changes. We apologize in advance for this inconvenience and will make every effort to accommodate re-scheduling your appointment.
Sick Patients and Children Please call before your appointment if you or your children are ill so we can make special accommodations for you. We appreciate your discretion as it affects the health of our pregnant patients and their babies.
